While reviewing records at the Primate Center it was noted that since July 2014 there have been two incidents of non-human primates escaping due to technician error that led to injuries requiring surgery (partial digit amputations) or primary closure of wounds. Two animals were affected. Also, it was noted that there have been three incidents of mis-pairing non-human primates due to technician error that caused injuries requiring primary closure. Four animals were affected. In order to prevent further escapes and mis-pairing of animals, the research facility must ensure that personnel always handle animals as expeditiously and carefully as possible. Note: The facility previously identified these incidents and has implemented corrective actions to prevent and minimize further incidents. Correction: Ensure corrective actions are properly followed. This was a routine inspection of CALS and the Veterinary School and a focused inspection at the Primate Center conducted on 1/21/2015 through 1/23/2015. ***This facility has made significant progress to correct the perimeter fence. Maintenance personnel were present and met with USDA inspectors during this inspection. A written work order is in place for the remaining areas of concern. The facility must complete the following improvements to fully correct this non-compliant item: ** The manure holding area has walls that are approximately 4 feet in height and made of concrete. These walls are a potential leaping point should an animal try to get over the fence. The facility plans to raise the height of the perimeter fence in this area to be 8 feet above the walls. The higher perimeter fence will also extend 4 feet on both sides of the walls. ** There is one storage building that is immediately next to the perimeter fence in the maintenance area that has sidewalls that are not 8 feet in height. Animals could potentially jump to this building and then over the perimeter fence. The facility plans to raise this building so the side walls are 8 feet in height. ** There is still building materials in the maintenance area next to the perimeter fence that need to be removed. These materials are a potential point where animals could use them to get over the perimeter fence. The perimeter fence must be kept in good repair and items kept away from the fence to ensure it is functional. There were several metal feeders with edges that appeared jagged and sharp on the side facing the inside of the chinchilla cages. It was explained that some of the chinchillas chew the feeders. There was also a small to moderate amount of rust on the insides of these feeders. Extensive damage to the surfaces of the feeders, leading to jagged edges or rusting, prohibits proper cleaning and sanitation of the receptacles. This is because pathogens may reside in a damaged or porous surface. In addition, rust can flake into the animal feed and may harm the animals if it is ingested. Animals may also injure themselves on the sharp edges of the feeders. Food receptacles shall be kept clean, sanitary, and free from contamination to protect the animals from disease and injury. One male red kangaroo, approximately 2 years old, is repeatedly circling to the left with a mild head tilt to the left. Both eyes are moderately protruding from the head. There is a light pink soft tissue structure approximately 1cm in size present at the inside corner of both eyes. Failure to seek medical care for animals showing signs of illness or injury can lead to unnecessary pain and discomfort for the animals or further veterinary complications. The animal listed above must be examined by a licensed veterinarian in order to ensure that an accurate diagnosis is obtained and an appropriate treatment plan is developed and followed. This information, including the diagnosis, treatment and resolution of the condition, must be documented and made available to the inspector upon request. There are four outdoor enclosures that have severely rusted cage doors. The rust is pitted and flaking. On each door, the rust has eroded through the metal surface creating large gaps and holes in the bottom rails on the doors. There are four dogs in the affected enclosures. Rust can affect structural strength and prevents the required cleaning and sanitization of surfaces, which can lead to disease hazards for the dogs. All rusted surfaces that come into contact with the dogs must be repaired or replaced so they can be properly cleaned and sanitized and to ensure structural strength. In the outdoor housing enclosures containing fifteen dogs, the food or water receptacles are made of hard plastic. These receptacles have been chewed and damaged by the dogs and have a buildup of dirt and grime in them. There is a wall along the back side of the enclosure, below the large overhead structure, opposite from the ocean, that has significant deterioration along the bottom edge where it meets the water. It appears that pieces have become detached, in which case they would fall into the water. The deteriorating section is discolored, with brown, green, and black areas. Part of the wall is painted, and edges of the painted wall are missing. The dolphins have access the this deteriorating section of wall. Debris falling into the water could introduce contaminants, pieces that could be ingested, and also makes adequate cleaning of surfaces difficult. All surfaces in a primary enclosure must be constructed of durable, nontoxic materials that facilitate cleaning, and disinfection as appropriate, sufficient to maintain acceptable water quality. All surfaces must be maintained in good repair as part of a regular, ongoing maintenance program. To be corrected by 7/1/15. An exit briefing was conducted with facility representatives.

There are two horizontal support beams across the middle portion of a geriatric lion's enclosure. An area of this support beam has separated resulting in a gap. It is large enough to allow an unauthorized animal to enter the enclosure and/or may lead to continued separation of the beams if not repaired. The gap should be reduced to minimize the risk of injury to the animal. The facility must be constructed of such material and of such strength as appropriate for the animals involved. Close inspection of the dog's mouth revealed that there was a thick brownish colored material on the surface of the teeth encasing several cheek and canine teeth causing it to be difficult to visualize the normal shape of the teeth. Dental problems can make it difficult for an animal to eat normally, can be painful or can lead to other systemic health problems. ***A female adult Shih Tzu (microchip # 096 362 521) had long fur around its eyes making it difficult for the inspectors to visualize the eyes. Close evaluation of the dog showed that the left eye was covered by matted fur. After the fur was pushed aside, it could be seen that the eye was dry and had crusty discharge. Irritated eyes can be painful and lead to additional eye issues. There is no written documentation to demonstrate that these dogs have received care regarding these health concerns. Additionally, the facility's Program of Veterinary Care does not have a plan in place to address these health issues; therefore, an evaluation is needed by the attending veterinarian to provide adequate veterinary care to these dogs.

On January 7, 2015, three cotton top tamarins were left in an outdoor enclosure overnight. They did not have access to indoor heated enclosures.The recorded temperature was below freezing and presented a threat to the animals' health and well being. The exposure to these extreme climate conditions were the cause of death for two of the tamarins. When weather conditions present a threat to animal's health or well being, appropriate measures must be taken to alleviate the impact of these conditions. These dog doors were located in the Main kennel building in several enclosures housing 10 adult dogs. The top row of enclosures has two enclosures, holding no dogs, with an accumulation of fecal waste on the vinyl wire flooring. These enclosures where located next to enclosures housing dogs. * The outside concrete flooring for the Little Kennel, housing 13 adult dogs, has brown to black grime smeared onto the surface. The lower metal pipe framing for the chain link dividers in these enclosures has an accumulation of brown to black waste smeared onto the surface. The inside enclosures for the Little Kennel building have an accumulation of brown to black waste in the corners of the enclosure boxes and smeared on the surface to the dog door inserts. The mentioned surfaces indicate that daily spot-cleaning is not taking place. Improper cleaning and sanitization can cause an accumulation of waste, attract pests, cause odors and increase disease hazards for the dogs. All surfaces must be spot-cleaned daily and must be sanitized according to Section 3.11(b) (3). Proper cleaning and sanitization will reduce the disease hazards for the dogs. There is a container of FastTrack Liquid with an expiration date of 02/2012 and several other expired medications. There are two unlabeled syringes containing a dark liquid material stored in the food storage refigerator. One Bag of Horse Strongid-C pellets that the licensee stated she is feeding to the hoof stock has an expiration date of 06/2012. The bag of feed and medications were found inside the commissary intermingled with other items being provided to the animals, and therefore are considered ready for use. Expired and unlabeled medications can experience spoilage or have reduced efficacy. Use of expired and unlabeled medication could lead to prolonged illness or suffering for the animals needing the medication. The drugs that were observed by the inspectors currently on hand, are being used off labeled for the species involved. There was no written approval for use by the attending veterinarian or any dosage instructions for the proper use of these drugs. Using drugs in an extra label manner may be harmful to the animals if not administered appropriately and under the direction of a veterinarian. Ensure that all medications used in the facility are not expired, are labeled properly in accordance with standard veterinary practices and are approved for use in the animals present in the facility by the attending veterinarian. The plan, at a minimum, must address social grouping, environmental enrichment, and special considerations (such as juveniles, those showing signs of psychological distress and individually housed nonhuman primates). The presence of these droppings indicate that these surfaces are not being cleaned and sanitized on a regular basis. There are several places throughout the facility where the animals' enclosure fences are in need of repair: -The lower section of the white-tailed deer fence is curled up in towards the enclosure. Also there are places where this fence is broken resulting in wire points. -The lower section of the adult addax enclosure fence near the feeder is exposed with wire points present. -The material covering the enclosure fence of the young addax has been ripped in places resulting in gaps and holes between this material and the enclosure fence. -There are wire points in the enclosure fence of the zeedonk enclosure. -There are several wooden posts that are attached to the steel fence of the camels' enclosure that have been broken off. This has resulted in holes in this fence that are two to three feet tall. -The two camels in one of the north enclosures have leaned over the top of their enclosure fence in several places. This has resulted in that fence being been pushed down from its original height to now being only a few feet tall. Two metal building panels of the shelter for two camels in one of the north enclosures have been kicked in. This has resulted in these panels bowing out into the enclosure exposing metal edges to the animals. Enclosure fences and housing facilities that are not kept in good repair can result in injury to the animals or may not securely contain the animals. All building materials must be constructed of materials that are structurally appropriate for the animals and must be maintained and kept in good repair to protect the animals from injury and securely contain the animals. Correct by 22 April 2015.

Salt block and hay is being fed directly on the ground for one male white-tailed deer. The hay was contaminated with excreta and the animal was observed walking through the hay. The pile of hay with the excreta is unfit for consumption. Contamination of feed can lead to intestinal parasitism or overgrowth of microorganisms which pose a risk to the health of these animals and feeding off the ground reduces this risk. Food must be wholesome, palatable, and free from contamination and of sufficient quantity and nutritive value to maintain all animals in good health. The sale slips did not include the name and address of the buyer. Heating. At the start of this inspection there was no heater present in the guinea pig room. Temperature in the room was 48.8 degrees Fahrenheit without a heater present. The guinea pigs were huddled together in the corners of their enclosures in order to stay warm. The owner brought in a propane tank with a gas burner on top during the inspection. The temperature rose to 54.7 degrees Fahrenhiet with the heater on after 30 minutes. A safe and reliable heater must be installed in the guinea pig room to prevent the ambient temperature from falling below 60 degrees Fahrenheit. Animals may become ill if the temperature is not closely monitored and kept betweem 60 - 85 degrees Fahrenheit. The licensee must ensure that the temperature is maintained at acceptable levels regardless of changing conditions outside. The ambient temperature must not fall below 60 degrees Fahrenheit in the guinea pig room to protect the health and well-being of the animals. Correct from this day forward.

Two animals were noted during the inspection to have conditions requiring veterinary care: **A Barbados ram lamb was noted to hide behind one of the ewes, his left hind leg appeared to be at an abnormal angle, and he wasn't bearing full weight on the leg. The licensee stated the lamb had a broken leg. He stated the attending veterinarian (AV) had seen the lamb a couple of days ago and stated "there is nothing to do about it". There is no documentation from the AV or the licensee to show when this animal was noted to have a problem nor what was to be done to provide relief from the painful condition. **An adult Dorsett ewe was noted to have the left nostril partially closed and pinkish tissue around the nostril and below the left eye appeared to be swollen. The licensee stated that the animal was old and may have a "sticker or something" that was causing the misshapen or swollen face and that he planned to take her to an auction next week. No diagnosis, treatment, or plan to relieve the painful/distressful condition had been made. Limb fractures are painful and should be appropriately diagnosed and treated under the direction of the AV to minimize pain and distress. Facial swelling and obstruction of the nares can be painful and/or distressful and should be appropriately diagnosed and treated under the direction of the AV to minimize pain and distress. The licensee shall have and follow a program of veterinary care for all regulated animals that includes the use of appropriate methods to prevent, control, diagnose, and treat diseases and injuries These two animals need to be seen by a veterinarian by 1/23/2015 and the licensee must provide documentation to APHIS officials regarding the diagnosis, any treatment prescribed and to show that treatment is given as directed, or that other methods for eliminating pain and distress associated with the diagnosed condition, and a plan for follow-up to determine if treatment is successful.
