In July 2014 a rabbit was anesthetized for two minor procedures, one of which was a partial closing of the eyelid, which requires post- procedural analgesics and 2 days of special monitoring. Two days after the procedure the animal care staff noted the animal was injured and provided veterinary care at that time. Upon contacting the investigators they were informed of the procedure. Had the investigators done proper post-procedural care which includes notifying the animal care staff of the procedure verbally or with post-operative forms that document their observations and the provision of analgesia the injury may have been noticed in a timely manner and prevented additional pain and/ or distress to the animal. The facility has taken steps to prevent future recurrence of this by retraining the investigators and implementing an augmented communication and documentation process. This item has been addressed by the facility.

Social grouping. There were two sub-adult female macaques housed singly at the time of inspection. They had previously been housed together and had been deemed compatible. During a housing change 2 weeks ago they were separated into individual enclosures. They were not under a veterinary or IACUC exemption from the social housing requirement. The facility must ensure that the social needs of non-human primates known to exist in social groups in nature are provided for the health and well-being of the animals. ***There was a spotted Ossabaw pig (#160-5), approximately five months old, which had lost four pounds in one week. The animal was weighed by the caretaker on 8/12/14 with a recorded weight of 29 pounds and on 8/19/14 the animal(cid:25)'s weight was recorded at 25 pounds. The animal had a large amount of food present in its bowl, while the other three pigs at the facility had empty food receptacles. According to the caretaker the animals were all weighed weekly and were each maintaining weight or gaining weight. This amount of weight loss in a growing animal could indicate an illness, infection or other veterinary medical condition. The registrant must have this animal examined by a veterinarian in order to obtain an accurate diagnosis and ensure that an appropriate treatment plan is developed and followed. The registrant must ensure that all animals receive adequate veterinary care at all times. TO BE CORRECTED BY: August 23, 2014. The Dalmatian Pen: The entrance floor of the shelter has torn carpet. The dog has been chewing and shredding the rug. Primary enclosures for dogs have to be designed and constructed of suitable materials so that they are structurally sound and will not potentially create a health hazard. Licensee stated that euthanasia by gunshot was the routine form of euthanasia for the facility. Licensee also said that they were under the impression from their veterinarian, that it was acceptable and he had written it on the old Program of Veterinary Care, which the inspector did not see. The current Program of Veterinary Care states that a lethal injection by the veterinarian is the facilities routine form of euthanasia. The inspector explained to licensee that gunshot is an unacceptable form of routine euthanasia. Euthanasia by gunshot can cause unnecessary pain and distress to the dog. According to the 2013 AVMA (American Veterinary Medical Association) Guidelines for the Euthanasia of Animals, gunshot is not an acceptable routine form of euthanasia for dogs. The licensee agreed to stop using gunshot and will use lethal injection by veterinarian as the facilities routine form of euthanasia. Correction date: Immediately There are multiple bottles and vials of expired medications and vaccinations stored in a small refrigerator along with vaccinations for the dogs that are still of an acceptable date. Expired medications may lose their effectiveness and cause the health of the dogs to suffer. All expired medications should be discarded or removed so they will not be used on the dogs. Between the outdoor enclosures housing three cocker spaniels and six cocker spaniels there is a metal pipe running along the floor that fencing is attached to. The pipe is starting to acquire excessive rusting. The rust causes pitting and flaking of the metal and does not allow for proper sanitization of the pipe. - There was standing water and a large amount of waste and old food in a drainage ditch along one entire exterior side of the primate medical building. The standing water is a result of the waste removed from the inside portion of their enclosures. There are drain pipes from the indoor portion of the enclosures that drain into an approximately 2 foot wide area directly behind the medical building. The primate shift area is directly above this drainage area, and the outside portion of this primate enclosure is adjacent to this standing water. This is essentially an open sewer, and is present due to facility employees not maintaining this drainage ditch in a proper manner. Improper drainage systems and standing water where the animals are contained may result in noxious odors, spreading of diseases, insect infestation and other health hazards. All pens housing animals must have an adequate drainage system that is able to eliminate standing water and waste more rapidly for the overall health of the animals. Corrected during inspection.

In the outdoor enclosure housing a single spider monkey "Ricky" the metal roof surface had an area of rust which had worn through the surface and created a hole with sharp edges This area had been patched with two pieces of sheet metal which were loosely attached creating gaps and exposed sharp edges. This creates a risk of entrapment for the enclosed animal, as well as a risk of injury from the sharp edges. The facility must repair or replace this section, and must maintain metal surfaces throughout the facility to protect the welfare of the animals.

- Numerous wasps were observed on a large nest located on the inside roof supports in a Non Human primate enclosure housing "Ruthie" and "Lucy". The nest was directly over a perch area, and insects were also flying in and around the enclosure. Wasps can sting and cause injury to the enclosed animals. - In the rear of the park, an enclosure containing 4 wolves has a grate floor that is exposed along three of the sides. This exposed grate flooring has large 4 inch holes that create a possible injury threat to the enclosed animals. Licensee needs to remove the grating or replace dirt on top of the grate to ensure the animals are safely contained and protected from potential injury. - In a pen in the rear of tha park containing tigers there is an area in the lower hinge side of the holding pen gate that has rusted through. Facilities need to be maintained in good repair. Licensee must repair or replace this gate. - In the rear of the park in the fence panel between cages 120 and 125 there is a pole that has significant rust present, to a degree that may affect the structural strength of the pole. Licensee needs to repair or replace this pole to ensure the animals are safely contained and protected from potential injury.

- The grizzly bear enclosure contained no water. There were no drinking water receptacles present in the enclosure and the play water receptacles were empty. The licensee stated that the clips on the drinking water receptacles were being replaced and that the containers had been removed from the enclosure around 9 am. It was around noon and the temperature was around 82 degrees. All three grizzly bears were panting. The licensee turned on the water hose and two of the bear drank aggressively from the water hose. The licensee brought in two small water receptacles. Licensee was instructed that due to the size and type of animals present those were too small and would not hold enough water for grizzly bears on high temperature days. Water must be supplied to the enclosed animals as often necessary for the health and comfort of the species of enclosed animals.

- A tiger enclosure in the back portion of the park contained rancid meat on the floor of enclosure. At least one piece of the meat was covered with numerous maggots. Food not consumed must be removed from primary enclosures as often as necessary to prevent contamination of the animals contained therein and to minimize disease hazards and to reduce odors. - There was a large piece of meat draped over the enclosure fence in a tiger enclosure in the front part of the park and a piece of old meat trapped in the feed chute in a tiger enclosure that was next to the camel enclosure. On Tuesday August 19th, 2014 two inspectors from USDA/AC attended the 6pm 'Tiger Playtime' at the facility. During this encounter up to 30 members of the public are allowed to sit in an area surrounded by fence panels under a tent where two tiger cubs (said to be 14 weeks of age and approximately 25-30lbs) were let into the room to interact freely with the adults and children present. During the show, the licensee was in the area for the first part of the event with one assistant. When the Licensee left, another assistant entered the area. Before the release of the white tiger into the room, the licensee aggravated the tiger by grabbing her by the scruff and bouncing her up and down on his lap and ground while the tiger hissed, growled, and bit at his glove covered hands. The tiger's ears were pinned to the side of its head during that part of the encounter and was not acting in a relaxed manner. As described by the Licensee, the tiger was "pissed off". While the tiger was agitated, the licensee dropped the tiger in the lap of the unsuspecting member of the public sitting next to him. The tiger jumped off this man's lap with its ears still pinned to its head. The tiger then walked away and flopped onto the floor. At this time the tiger's ears returned to a normal un-agitated position and another tiger was released into the room. The tigers walked, played, and jumped around on many members of the public. People were lying down next to them, petting them and taking photos with them during the length of the event. There were three incidents where members of the public were scratched or bitten during the play time. One older female was pawed and mouthed in the face by the white tiger. A young boy (estimated to be approximately 10yrs old) was bitten on the top of his thigh. The child did yell out, pushed the tiger off of his leg, and crawled away rubbing his thigh. One of the assistants asked him if his leg was still 'attached' and teased him about his leg being chewed like a drumstick. The boy continued to pet and play with the tiger. Another bite was to the back of one of the inspectors, which resulted in broken skin and bleeding. Another young boy was approached from behind by the tiger where it grabbed both sides of his head at the ears and mouthed the top of his head. In another encounter a tiger was lying across the back of a female lying on the floor while chewing and pulling on the hair of another woman sitting next to them. There were several instances where members of the public would rile up the tigers by rough housing with them, or shaking their heads by gripping their faces, and when the tigers would respond in play by swatting with their claws or biting their hands, multiple people responded by 'swatting' the tigers in the face, nose and eye area, or prying their mouth off of their hands or arms with their free hand. There were several instances where the tigers would leap on top of people lying down, or jump up onto the hips and lower backs of a standing person. Due to the weight and size of the tigers, the teeth and claws could cause damage and several members of the public did say "Owe or ouch" during the play time as the tigers swatted, jumped and mouthed the people. While the tigers are young and at play with members of the public, the current procedure of letting the tigers walk freely among so many people with no direct control by the keepers in the area provides an immediate opportunity for injury to the public by the tigers' sharp claws and teeth. There is also a risk to the tigers due to members of the public providing a 'discipline' response by swatting, pushing or prying off of their mouths. This action by untrained members of the public teaches and encourages the tigers to become more aggressive in their play and may result in the tigers becoming more unruly and unresponsive to discipline. Interactions between the tigers and public could be managed in a controlled manner under the guidance of a trained keeper, but was not observed by inspectors during the Tiger playtime they attended. Free roaming contact and unregulated discipline given by the public provides opportunity for an injury to an animal or the public before control of the situation could be gained by a trained keeper. After the "Tiger Playtime" at 7pm there was a show with multiple animals where the public was given the opportunity to see and handle other species. The licensee had many of the animals leashed and talked about them while the public observed. On one occasion a capuchin monkey was tossed onto the lap of a man next to the inspector. The monkey was on a long leash and was swung from the belt that was around its hips and tossed approximately 6 ft onto the man's lap. Another monkey was twirled around the licensee by it's belt and leash. The lemur was allowed to jump onto people causing surprise reactions by the public. A young coati was allowed to roam free across people's laps, and a young kangaroo was allowed to roam free in the event area. The public was allowed to hand feed many of the animals. Animals may be injured or harmed when interaction with the public is free and uncontrolled. There needs to be more control of the environment and animals when they come in contact with the public. All animals must be handled so there is minimal risk of harm to the animal and to the public during exhibition, with sufficient distance and/or barriers between the animal and the general public so as to assure the safety of animals and the public.

The issue with unsealed wood continues with the three domestic dog-hybrids (three wolf-dog hybrid and two coyote-dog hybrids) that continue to be housed in a sheltered enclosure towards the bear enclosure. The sheltered portion of the primary enclosure housing these dogs is constructed of unsealed wood which is not impervious to water with support posts that have been chewed. The dirt floors under the sheltered portion of the enclosure do not have access to direct sunlight and can not be sanitized. Walls and flooring constructed of unsealed wood and dirt which are permeable to moisture provide an optimal area for bacterial and fungal growth both of which can cause disease in the dogs housed in these enclosures. Ultimately the failure to construct dog enclosures out of surfaces that are impervious to moisture results in an inability to properly clean and sanitize the primary enclosures and creates an increased risk of disease and illness. The new enclosures are still being scheduled for completion that will house all wolf-dog and wolf-coyote hybrids according to AWA standards and regulations. The licensee advised that the current enclosures will remain as is until the animals are moved to the new enclosures. Failure to follow through with the planned repairs or new buildings that meet the AWA standards and regulations will cause the animals to continue to be housed in a structure that is not impervious to moisture and can not be sanitized due to chewed wood or unsealed cement.

In the 2 enclosures that house five dogs (three wolf-dog hybrid and two coyote-dog hybrids) are housed in a sheltered primary enclosure towards the bears continue to have an accumulation of more than 2 days of fecal material and hair. The accumulated debris is evidence that current cleaning and sanitation protocols are inadequate to prevent their accumulation. Cleaning is still not being done at a rate to decrease accumulation of excreta, urine soaked areas or food waste. The meat that was put into the enclosure had an accumulation of fly eggs due to the heat and moisture over the past 2 days, and has not been eaten or the left over waste removed. Accumulated organic debris provides an optimal area for the growth of bacterial and fungal pathogens creating a disease hazard for the dogs. Additionally, this accumulated debris can attract pests including flies and vermin as well as contributes to odors within the facility. The licensee must increase cleaning of fecal material to be as often as necessary to prevent accumulation of dirt, debris, excreta and food waste and sanitizing at least once every two weeks.

There are 3 original enclosures from the last inspection that are still in violation due to fencing being less than 12 feet high, as construction of the new enclosures are still in the planning process. The three large felid enclosures contain a total of 2 Tigers and 2 Lion and 1 dog. The remaining three affected enclosures that need correction are: Tiger Pen 4: contains one Tiger referred to as Nahandi Tiger Pen 5: contains one Tiger referred to as Glacier and one lion known Ungowwa Lion Pen 1: contains one male lion referred to as Cheif and Bandit(dog) None of these remaining pens have any angled top fencing (kick-in) or any species appropriate high tensile smooth electrical wire to provide additional deterrents for escape. In the Bear enclosure (four adult bears) with the pond, there were still several broken pieces of a wooden walkway along the fence with protruding nails(1-2inches) that was moved from the worn pathway into the grass along side of it. The bear pacing near the fence was avoiding the grass area where the wood with nails way lying, however it still poses a potential for injury as now the nails are not as well seen by the animals. Exposed nails and broken pieces of wood can be sources of injury and infection if the regulated animals scratch or puncture themselves on the materials. The reported enclosures(Tiger 4,5 and Lion 1) need to be built according to standards and regulation as currently they are not tall enough to properly contain the animals as these adult tigers could easily jump/climb out of the enclosure if they were motivated to do so. An escape places the animal's life in jeopardy and may endanger the safety of the public. All broken pieces of wooden material with exposed nails in the enclosure shall be removed and/or repaired to prevent any injuries to the regulated animals.

There were still several Tiger water pools (enclosures' 1, 2, 3, 4 and 5 ) that were green with algae, including the bear pond with (4 bears), 2 cougars and the 1 lion and dog(Bandit). The water pools had floating clumps of algae in some areas or a coating of algae on the bottom or sides of the metal tubs/pools. The black leopard had green floating algae in his water tank that has not been cleaned in 6-8 days. The 8 foxes, 1 cougar and 2 porcupines had floating algae on all of their water buckets. Algae buildup can cause the water to be a source of harmful organisms that can be toxic to the animal and cause possible health issues. The buildup of algae can cause skin disorders and is not a suitable drinking source. The licensee must clean the water pools and water drinking sources as often as necessary to prevent the buildup of algae. Inspection and exit interview was conducted with the licensee and his wife. 1.There is a male Japanese Chin type dog approximately 10 years of age named 'Ki Jack' that does not have official identification. The dog has an accumulation of greenish-colored mucus covering both eyes. Not having clear vision can cause discomfort and affect the health and well being of the dog. The presence of eye discharge can be a sign of injury, infection or other potentially painful veterinary medical conditions. The potential for injury exists if the dog is not able to see clearly. The dog was also observed having matted hair on his abdomen. The matted hair area is approximately 1 inch wide and 4 inches long. Excessive matting causes pain and discomfort to the animal. In addition, matting can foster skin infections and can hinder the animal's ability to move of defecate normally. The licensee needs to remove the matted fur on the dog. As part of the facility's program of veterinary care, the licensee must establish and follow a program of regular coat maintenance for all of the dogs to prevent the development of excessive matting. On the dog's nose there is a crusty appearance of scabbing caused by numerous fly bites. Fly bites to the nose can lead to open sores which can cause further disease risks or infection for the dog. Left untreated it could delay the healing process affecting the health and well being for the dog. Fly bites need to receive veterinary care to prevent further fly biting and then the nose can start to heal. 2. There is a tan colored male cocker spaniel named ' Poo Bear' with no official identification. The dog has an open wound on the back of its neck. The wound is approximately 3.5 inches in length and 1.5 inches wide. There is dried blood on the wound and around the edges in the fur coat. The cause of this injury is unknown. The open wound can be painful causing discomfort and could lead to the spread of infection which affects the health and well being of the dog. Around the dog's nose there is a sore caused by fly bites that is open and areas where the wound is starting to appear dried and crusty. Pieces of fur are stuck to the moist portions of the wound. Fly bites to the nose can lead to open sores which can cause further disease risks or infection for the dog. Left untreated it could delay the healing process affecting the health and well being for the dog. Fly bites need to receive veterinary care to prevent further fly biting and then the nose can start to heal. 3. There is a male golden retriever named 'Henry James' with an open wound approximately 3 inches in diameter on top of its left ear. The wound appears moist with a bright red and pink color. There is a hairless area outlining the wound. The licensee believes a pickup camper shell that blew into the enclosure during a storm struck the dog causing the wound. The open wound can be painful causing discomfort and could lead to the spread of infection which affects the health and well being of the dog. *The licensee must have the above animals examined by a veterinarian to obtain an accurate diagnosis and appropriate treatment plan. The majority of the housing facilities have an excessive accumulation of exceta on the floor where the dogs are in contact with it. There is a mixture of fresh and old fecal residue that has dried out and appears white covering at least 60 percent of available surface flooring. A mixture of dirt, grease and other excreta are on the bottom portion of the interior and exterior walls of the housing facilities. Cleaning and sanitation of surfaces that are in contact with the dogs is necessary to prevent an accumulation of dirt, debris, excreta and other disease hazards. Unsanitary living conditions and potential transmission of diseases exist when surfaces are not cleaned and sanitized. Adequate Veterinary Care A female Corgi (microchip number 003814847) had excessively long nails to the point that they were beginning to curl. Overgrown nails can result in abnormal gait, broken nails or lameness. Trim the nails of this dog and maintain appropriate methods of care such as regular nail trims to prevent them from being overgrown and to provide for the dogs' overall health and well-being. A male Poodle (microchip number 087340601) was observed to have soaked fur around its mouth. Closer inspection revealed that a dark brown material was adhered to the surface of all the teeth. The upper and lower cheek and canine teeth had a gray colored creamy material along the gum line extending down into the spaces between the teeth. The upper left cheek teeth had a visible separation between the gums and the teeth. Failure to properly identify and treat dental health issues can lead to a poor state of health and nutrition by causing pain or leading to loose teeth, abscesses, and bone loss or infection. This dog must be examined by the attending veterinarian to receive veterinary care for this dental health issue. Documentation must be maintained by the licensee on the veterinary exam and treatment provided to this dog.

Condition and site There is a large round fan used for ventilation hanging from the ceiling on the left hand side of the housing facility as well as a ventilation fan on the ceiling in the middle of the building that both have a significant accumulation of animal hair, dust, cobwebs, and brown matter covering them. The enclosures under and around the large round fan have the same buildup on the tops of the wire enclosures. The enclosure measured 24 inches by 36 inches with food and water receptacles taking up .5 square feet of space and therefore providing a total of 5.5 square feet of floor space. In June, 2014, Koko, the 43 year old female gorilla, experienced an episode described in her medical record as: "has not been doing well for the past week- has not been moving much, cannot seem to use legs well, poor appetite, lethargic." The owner and the attending veterinarian, who is out-of-state, speculated over the phone/text/email on various causes for her ailments, including stress/psychological and/or arthritis. The vet notes that the owner declines an immobilization for a full work-up. A pharmaceutical regimen is prescribed, and within the next few weeks, after adjusting the drug regimen by phone/text/email, Koko improves. During this episode, where Koko was described as being unwilling to use her legs, and also, according to on-site records, experiencing reduced function of her left arm, Koko was not examined by a veterinarian. There could have been many possible, and very serious causes for her sudden decline. Failure to conduct a physical exam put the animal's well-being at risk. The exhibitor is expected to establish and maintain a program of adequate veterinary care that includes the use of appropriate methods to diagnose and treat diseases and injuries, and the availability of emergency care. Serious clinical signs, such as those exhibited by Koko during this time period, should be promptly evaluated by a veterinarian via examination of the animal on site. If the attending veterinarian is not available, then a local emergency veterinarian should be consulted, as per the written program of veterinary care. A Veterinary Care issue was previously cited in 2012.

The kennel operator may have to move these dogs into an area that is free from flies until the healing is complete. The owner must have these two dogs examined by a veterinarian to help get these open wounds healed, set up a program of treatment for both dogs and administer the appropriate medication. The kennel operator must provide a treatment record and or record of consultation with the veterinarian upon request during the next inspection. ***There was insufficient distance and/or barriers between the tigers and the public during the time that the public was allowed to feed the tigers. The public is allowed to use kitchen tongs to feed raw meat to the tigers while having their photo taken by an attendant. The barrier that the public stands behind was about 3 feet high and was 32 inches away from the tiger enclosure. This height and distance was not enough to prevent people from leaning over the barrier far enough to potentially be able to stick their fingers or hands into the 2 inch by 4 inch openings of the enclosure.
