There is a written program of veterinary care but the last visit by the veterinarian is greater than one year ago on March 13, 2014. Regular inspections by attending veterinarians are required in order to ensure adequate veterinary care at the facility. The licensee shall employ an attending veterinarian under formal arrangements and must schedule regular visits to the premises. To be corrected by July 15, 2014

At least four animals did not have any form of official identification. Individual identification is required in order to ensure that each animal's identity can be known with certainty and to facilitate record keeping requirements. All live dogs on the premises must be officially identified with a tag, tattoo or microchip and this identification information must be made available to the inspector. To be corrected by July 15, 2014

Surfaces. At least one shelter has visible rust. The rust is pitting and flaking along the bottom portion of the shelter. In some places the metal has completely eroded leaving holes and jagged edges along nearly the entire bottom edge of the shelter. Rusted surfaces that are pitting and flaking cannot be cleaned or sanitized properly. It can also affect the structural strength of the shelter. Interior surfaces and any surfaces that come in contact with the dogs must be repaired or replaced so that it is free of excessive rust that prevents the required cleaning and sanitization, or that affects the structural strength of the surface. None of the shelters have wind and rain breaks at the entrance. One of the shelters contains water on the floor. Lack of wind and rain breaks does not provide protection from the elements. Shelter must be provided with a wind break and rain break at the entrance. To be corrected by July 15, 2014

Dust, dirt and grime are present on most of the surfaces in the primary enclosures. These surfaces include all of the food, water bowls and shelters. Some of the dust, dirt and grime are present inside the food and water bowls. There is a buildup of fecal material mixed in with the dirt and gravel flooring. There is a mixture of old and fresh feces scattered throughout some of the enclosures. Failure to sanitize and clean the enclosures can lead to disease hazards for the dogs. Used primary enclosures and food and water receptacles for dogs must be spot cleaned daily and sanitized at least once every 2 weeks using one of the methods prescribed in paragraph (b)(3) of this section, and more often if necessary to prevent an accumulation of dirt, debris, food waste, excreta, and other disease hazards To be corrected by July 15, 2014

Pest control. An excessive number of flies are on and around a pile of wet feed that was spilled on the ground. The spilled feed is located next to a food receptacle in an enclosure affecting at least two dogs. An excessive number of pests can lead to disease hazards. An effective program for the control of insects, external parasites affecting dogs that are pests, must be established and maintained so as to promote the health and well-being of the animals and reduce contamination by pests in animal areas.
